1. Report all injuries to the athletic trainer as soon as possible.  Immediate and daily communication with the athletic trainer will expedite your recovery time.

2. All student-athletes must sign in on the “Daily Treatment Log” before receiving any athletic training services.

3. No over the counter medications will be administered.

4. Do not take supplies without permission from the athletic trainer.

5. Please return all equipment issued to you by the athletic trainer.  Failure to return items such as splints, elastic bandages, protective braces, and crutches will result in a fee charged to your school account.

6. Proper attire is required for treatment.  (Shoes, shirt, sneakers)

7. No socializing.  You should only be in the athletic training room if you are receiving treatment or need to be evaluated.

Every student-athlete is required to have a complete pre-participation physical examination that positively determines readiness for athletic participation.  This physical is good for a one-year period.  Athletes will NOT be allowed to participate in athletic practices or competition until a signed (signed by the health care provider) pre-participation physical exam is submitted to Magnus Health or a hard copy is given to the athletic trainer.   

Parents/Guardians must also sign the “Athletic Consent to Treat and Acceptance of Risk” form and “Athletic Concussion Information” form prior to participation in athletic activities. These forms are found on the student-athletes Magnus Health account. 

We understand that when an injury occurs you have many options on how to get your daughter properly evaluated and treated.  You are under no obligation to use our physician, however there are benefits in doing so.  If your daughter sees her personal or specialty physician, written documentation must be returned to the athletic trainer with the diagnosis, treatment plan, and any recommended restrictions and timeline for return to play. The Physician Referral Form can be used for documentation.  If your daughter goes to the Emergency Room for an injury, we must receive a written medical release or recommendation for her prior to returning to athletic participation.  

If your daughter is seen by a physician, proper written documentation must be submitted to the athletic trainer.  Your daughter will be held from practices and games until a note from the treating physician is received.

If a concussion is suspected, please contact the athletic trainer and school personnel.  If possible, we strongly recommend complete cognitive rest for the initial 24-48 hours after the injury.  This will require your daughter to remain home from school while symptomatic.   As tolerable she may return, with a note from the pediatrician, or concussion clinic explaining absence.  An ACE Care Plan should be completed by the treating physician and submitted to the school for each follow up visit until your daughter is cleared to return to academics without restrictions.
